The following steps must be completed before Scholars are able to participate ins sports:

  1. Register online at Final Forms: https://federalway-wa.finalforms.com/parents/login
  • Choose all the sports your Scholar may be  interested in.

2. Doctor completes the Physical Examination form: https://assets-rst7.rschooltoday.com/rst7files/uploads/sites/652/2023/06/14095551/PPE-Physical-Examination-Form-rev.pdf and Parent uploads to Final Forms https://federalway-wa.finalforms.com/?forward_path=%2Fparents%2F5784

  • Physicals are valid for two years and the original copy must be on file with the Athletic Office.

3. Parent completes the Eligibility Form: https://assets-rst7.rschooltoday.com/rst7files/uploads/sites/652/2023/06/14095547/PPE-Medical-Eligibility-Form-rev.pdf and uploads to Final Forms: https://federalway-wa.finalforms.com/?forward_path=%2Fparents%2F5784

4. Scholar completes online forms in Final Forms: https://federalway-wa.finalforms.com/?forward_path=%2Fparents%2F5784

5. Parent must complete All online registration form on Final Forms: https://federalway-wa.finalforms.com/?forward_path=%2Fparents%2F5784 before the scholar will be allowed to participate.

6. Scholar must maintain a grade of "C" or better in All classes in order to participate in interscholastic sports.

ASB Card

Students must purchase an ASB Card ($15) PRIOR TO THE FIRST COMPETITION. ASB Cards can be purchased with cash, check or credit/debit card in the main office or online with a credit/debit card at the FWPS Payment Portal: https://www.fwps.org/departments/business-services/pay-fees-online

Sequoyah Athletics 2026-2027

Season 1: 

  • Football (7th & 8th Grade Only): Starts September 2nd.
  • Boys & Girls Cross Country: Starts September 8th.
  • Fastpitch: Starts: September 8th.

Season 2: Starts November 9th

  • Girls Basketball
  • Girls Intramural Basketball (non-cut sport)
  • Boys & Girls Wrestling (non-cut sport)

Season 3: Starts February 1st

  • Boys Basketball
  • Boys Intramural Basketball (non-cut sport)
  • Gymnastics (non-cut sport)
  • Intramural Co-Ed Soccer (non-cut sport)

Season 4: Starts April 12th

  • Baseball
  • Boys & Girls Track (non-cut Sport)
  • Volleyball

Due to WIAA regulations, 6th graders are unable to participate in football.
